The project renovates an existing professional office building for use as a primary care facility for Community Health Center (CHC), completely reconfiguring the building to provide the model of care practiced by CHC in its primary care centers. The project provides primary medical care and behavioral health space organized into units of seven to eight client rooms each, all supported by Laboratory, Dispensary, Storage, and open office space for healthcare providers, and includes administrative space and a community room to support the primary care and behavioral health practices.
